Fetal growth restriction (FGR), formerly termed intrauterine growth restriction (IUGR), is an important cause of perinatal morbidity and mortality. This narrative review synthesises evidence on the role of Color Doppler and Doppler velocimetry in the diagnosis, surveillance, and risk stratification of pregnancies complicated by FGR, focusing on the umbilical artery, middle cerebral artery, cerebroplacental ratio, uterine arteries, and ductus venosus.
UA Doppler remains the most established fetal Doppler assessment, reflecting placental vascular resistance, while MCA Doppler may demonstrate cerebral redistribution (“brain sparing”) during hypoxic stress. The review concludes that Doppler assessment is integral to FGR surveillance, with UA Doppler as the cornerstone and MCA, CPR, uterine artery, and DV Doppler providing complementary information in selected cases, to be interpreted alongside fetal biometry and the wider clinical picture.
